Authorization to Obtain Consumer Credit Information
I, (print name), residing at , authorize Ash Ryben Consulting Inc. ("ARC") and/or a lender, broker, or credit-reporting reseller designated by ARC and identified to me, to obtain one or more consumer credit reports and credit scores about me from one or more consumer reporting agencies (Equifax, Experian, TransUnion).
Purpose
The report(s) will be used solely to evaluate my financial position and the credit-dependent strategies in my Financial Assessment or related engagement with ARC, which I have requested. I understand this is a business purpose I have initiated.
Type of inquiry
☐ Soft inquiry only (does not affect my credit score). ☐ Hard inquiry, where required by a lender to process an application I have asked to submit.
My understanding
- ARC is not a lender and this authorization is not a credit application. Any credit application will be a separate document with the lender.
- Information obtained will be kept confidential, used only for the purpose above, and shared only with professionals I direct ARC to work with, in writing.
- This authorization is valid for 90 days from the date signed unless I revoke it earlier by email to brad@ashrybenconsulting.com, and may be used for more than one pull within that period only if I initial here:
- I may request a copy of any report obtained under this authorization.
- Under the federal Fair Credit Reporting Act I have the right to know if information in my file has been used against me, to dispute inaccurate information with the consumer reporting agency, and to obtain a free copy of my report annually at annualcreditreport.com.
